Referrals for children ages 5–17

Peaceful Pathways accepts referrals for children and adolescents ages 5–17 who may benefit from short-term crisis residential stabilization. Admissions are based on clinical appropriateness, safety, medical necessity, authorization, and bed availability.

Who may refer

Referrals from families & professionals

  • Community Services Boards (CSBs)
  • Hospitals and emergency departments
  • Departments of Social Services (DSS)
  • CSA coordinators and FAPT teams
  • Managed Care Organizations
  • Schools
  • Courts and juvenile justice partners
  • Physicians, psychiatric providers, and licensed therapists
  • Families and guardians, when clinically appropriate
Admission criteria

Examples of when we can help

  • Emotional or behavioral crisis
  • Depression, anxiety, mood instability, or trauma-related symptoms
  • Aggression or escalating behaviors
  • Safety concerns that require structured stabilization
  • Risk of hospitalization or placement disruption
  • Need for medication review, stabilization, or coordination
  • Difficulty functioning safely at home, school, or placement
Referral process

Six steps from referral to admission

Contact admissions

Reach our admissions team by phone or through the online referral form.

Submit referral packet

Provide the referral and clinical documentation for review.

Initial review

Our admissions team completes an initial review of the information.

Determine appropriateness

The team determines clinical appropriateness and level-of-care needs.

Verify authorization

Insurance, funding, and authorization are verified as applicable.

Schedule admission

Admission is scheduled once approved and a bed is available.

Documents to request

Helpful information for review

Gathering these ahead of time helps our team review referrals quickly and accurately.

  • Current clinical assessment or prescreening information
  • Psychiatric evaluation, if available
  • Medication list and recent medication administration record
  • Recent discharge summaries, hospital records, or crisis evaluations
  • Custody / guardian documentation
  • Insurance or Medicaid information
  • School information, IEP/504, or behavioral documentation when relevant
  • Safety plan or recent risk assessments
